Parental Statement of Approval
In my capacity as sole legal representative / In our capacity as joint representatives
of the above-named minor, I/we hereby declare my/our explicit consent that the minor participates in the pre-medical course which is offered by CPE Europe GmbH, Albert-Einstein-Ring 11–15, 22761 Hamburg, Germany at its facilities at Universitätsmedizin Neumarkt am Mieresch Campus Hamburg (UMCH), Albert-Einstein-Ring 11–15, 22761 Hamburg, Germany between June 29 and July 17, 2020.
This parental statement of approval covers all activities and responsibilities related to the participation of the above-named minor in this course. It entitles the minor – in particular, but not exclusively – to engage in all legal transactions and to carry out all other relevant actions which are required for participating in the named course; such as (for example):
- Registering and enrolling in the named course
- Making the relevant payments for this course
- Using the services and facilities provided by CPE Europe GmbH
- Participating in courses, classes, seminars, etc.
- Participating in extracurricular/leisure activities
- Using laboratories and their facilities and performing experiments
- Participating in exams/tests
This parental statement of approval neither transfers the parental authority nor custody, supervisory duties, or any other kind of obligations or responsibilities for the minor to CPE Europe GmbH or to any of these two entities’ staff members, owners, representatives, etc.
The parental agreement of approval also comprises the concent of the minor’s parent(s)/legal guardian(s) that all notices, declarations, information, documents, etc. are directed to the minor. With the receipt of these documents/information by the minor, the submission is deemed to be completed and the respective documents are deemed to be received by the minor’s parent(s)/legal guardian(s).
Online Learning Platforms and Digital Enhancement of Teaching
It is an essential element of the teaching and learning strategy at CPE Europe GmbH to embrace digital and online-based approaches of teaching, to provide participants in the above-designated courses with access to e-learning platforms, and to enhance teaching by using digital media and programmes. This concept allows CPE Europe GmbH to offer courses which can be attended in two ways – either in person (directly on campus) or by using our online-resources (which allows them to attend from home). In order to provide our course participants with such opportunities, CPE Europe GmbH record select lectures (as well as extracurricular activities) which are made available to be re-watched by both our on-campus and online-participants. By making these recordings available to the participants and by allowing them to re-watch the taught content(s), their understanding of respective topics is deepened and the study success is significantly increased.
To carry-out this approach and to put it in practice, the parent(s)/legal guardian(s) allow(s) the minor to use all required software, the e-learning facilities, and all other kinds of digital media which are provided by CPE Europe GmbH for the purpose of participating in the aforementioned course. Moreover, this parental statement of approval also entitles the minor to enter all licence agreements with external providers of the required programmes.
The parent(s)/legal guardian(s) of the minor also entitle(s) CPE Europe GmbH to record (in form of pictures as well as video and sound recordings) lectures which take place in the context of the above-designated course and in which the minor participates. These recordings are created for the exclusive purpose of making them available via (and for permanently storing them in) the e-learning platforms of CPE Europe GmbH (such as Canvas, Panopto, Zoom, etc.) and other study-relevant media in form of live-streams and downloadable content. These platforms are fully protected, access restricted, and only registered participants of the courses, lecturers of the courses, and staff members of CPE Europe GmbH are given access to these platforms. The recorded materials are not made available publicly or in any form other than explicitly specified here.
The parent(s)/legal guardian(s) transfer(s) to CPE Europe GmbH the irrevocable and indefinite copyrights and rights to use, to store, to share, and to process the pictures, quotes, video and sound recordings of the above-designated courses in which the minor participates. Neither the parent(s)/legal guardian(s) nor the minor receive any copyrights or rights to use concerning the materials created in above-designated way and they agree not to charge CPE Europe GmbH for permitting these legal entities to produce the aforementioned materials, for transferring to them the above-designated rights to use as well as all relevant copyrights concerning these materials, and for giving the consent that CPE Europe GmbH are entitled to publish these materials in the determined way.
Liability
In the case of damages caused by the above-named minor – because of gross negligence or intent – to the life, health, or property of CPE Europe GmbH or any third party (such as fellow participants), the parent(s)/legal guardian(s) of the minor hereby declare(s) to assume liability.
Proof of Identity
As proof of their identity, the signer(s) submit(s) a copy/copies of their passport(s) or – alternatively – any other form of official identity document(s) with this statement.
